Use the form below to request records from the Avon Fire Department. We handle public records requests and requests for ambulance patient care reports differently because patient information is protected.
Public records
For fire incident reports and other department records, describe the records you are seeking and provide the date, location, or other details that will help us find them. We will review the request under Massachusetts public records law. Information that is confidential or exempt from disclosure may be withheld or redacted.
Ambulance and patient care reports
Ambulance reports contain protected health information and are not released through an ordinary public records request. Patients may submit a request for their own patient care report using the form below. The report will not be released until the patient comes to the Avon Fire Department at 150 Main Street, Avon, and staff verify their identification in person. Please call ahead to coordinate your visit; records may require time to prepare.
An attorney, law office, insurer, or other person requesting a patient’s report must provide a valid authorization signed by the patient or documentation showing legal authority to receive it. A request on office letterhead or this form alone does not authorize release. We will verify the requester’s identity and authority before releasing patient information.
Submit the request using the form below. For an ambulance report, identify the date and location of the response, the patient, and your relationship to the patient. Do not include medical details or attach sensitive records in the comments field. Email the patient-signed authorization or other documentation establishing legal authority to fireadmin@avon-ma.gov. Please include the patient name and date of the ambulance response so we can match it to your request. Additional documentation may be needed when requesting records for a minor, a deceased patient, or someone represented by another person.
Submit a request
The form accepts both public records and patient report requests. If a required incident field does not apply to a general records request, enter “Not applicable” and describe the records in “Type of Report Being Requested” or “Additional Comments.”
